Don't schedule pending change refreshes when the PC manager is inactive
and/or when a solution is loading. (When a solution load completes we
do a full refresh in one sweep)

* src/Ankh.Scc/PendingChangeManager.cs
  (OnSvnItemsChanged, Refresh, ScheduleMonitor, StopMonitor):
    Don't schedule specific refreshes when not active and/or loading solution.
